Cement-Retained Implant PFM Crown

Cement-Retained Implant PFM Crown

ement-Retained Implant PFM Crown (Porcelain Fused to Metal) is a classic implant-supported single crown. It is fabricated by cementing a PFM crown onto a stock or custom implant abutment, instead of fixing the crown directly by screw. The metal coping provides solid strength, layered porcelain delivers natural morphology and shade. Without screw access hole on the occlusal or facial surface, it achieves smooth, continuous crown contour. It is widely selected for anterior implant cases where screw channel would compromise aesthetics.

Description

What is Cement-Retained Implant PFM Crown?

A cement-retained implant PFM crown is an implant fixed restoration:

The abutment (stock or custom titanium) is first torqued and locked onto the implant fixture.

The PFM crown (metal coping + stacked porcelain) is then cemented onto this abutment with resin cement or resin-modified GIC.

No screw hole runs through the crown body.

Difference from screw-retained implant crown: Screw-retained crown has an access hole for retaining screw and can be retrieved easily; cement-retained crown is bonded on abutment and usually needs sectioning for removal.

Difference from natural-tooth PFM crown: It sits on implant abutment, not prepared enamel/dentin, and relies on abutment retention and cement seal.

Key Benefits

Superior aesthetic performance for anterior zone No screw opening on facial surface. Continuous porcelain surface, natural emergence profile, ideal for high aesthetic anterior implant sites.

High mechanical strength from metal framework CoCr or noble alloy coping resists fracture, suitable for heavy occlusion and bruxism patients. Good stability for posterior single implant restoration.

Flexible abutment customization We can mill custom titanium abutment to correct implant angulation, optimize soft tissue contour, create ideal shoulder margin for cement control.

Smooth occlusal design Occlusal table is fully uninterrupted, free of the screw hole filling material. Occlusal adjustment and contact tuning are easier.

Mature, predictable clinical solution Decades of clinical application, easy cementation workflow familiar to most clinics.

Wide alloy options CoCr, semi-precious alloy, high-noble gold alloy, titanium coping available per prescription.

Indications

✅ Anterior single implant, where implant angulation makes screw hole show on buccal side

✅ Posterior single implant with normal occlusal force, preference for solid PFM structure

✅ Cases requiring natural, seamless crown surface without screw access channel

✅ Budget-conscious implant restoration with high strength requirement

✅ Implant sites with adequate inter-occlusal space and good soft tissue condition

❌ Contraindications

Poor soft tissue access that makes excess cement impossible to clean completely (high peri-implantitis risk)

Cases requiring frequent retrieval and maintenance

Severe metal allergy (choose full ceramic implant crown instead)

Implant position with unfavorable emergence and subgingival margin too deep.

Frequently Asked Questions (FAQ) – Professional Q&A

Q1: Cement-retained vs Screw-retained implant crown, how to choose?

A: Cement-retained PFM: pick for anterior aesthetics, avoid visible screw hole; downside: risk of residual cement, hard to retrieve. Screw-retained: pick for easy maintenance, no excess cement risk; limitation: screw access hole may affect appearance.

Q2: Do you supply custom abutment together with the PFM crown?

A: Yes. We can deliver custom milled titanium abutment + cement-retained PFM crown as one set. Please mark implant brand, platform size and angulation.

Q3: What risk should clinicians pay attention to for cement-retained implant crown?

A: Extruded residual cement trapped subgingivally is the main hazard, which causes peri-implant mucositis and bone loss. We design crown margin at or supragingival level as much as possible to facilitate cement cleanup.

Q4: What files do you need for production?

A: Implant scanbody scan, antagonist scan, bite record, implant brand & platform info, abutment prescription.

Q5: Can this crown be repaired if porcelain chips?

A: Minor porcelain chipping can be intra-orally repaired; severe damage usually requires making a new crown, since the crown is cemented and hard to take off intact.

Q6: What cement type is recommended?

A: Resin cement or resin-modified glass ionomer cement is commonly used for cement-retained implant crowns. Avoid conventional phosphate cement.
